What you'll do

• Supervise staff engaged in appointment setting, reminder calls, care gaps, and assessments
• identify and communicate priority care gaps to clinic leadership and providers
• track, report, and evaluate care gap and referral outcomes
• develop and use dashboards and scorecards
• coordinate post-discharge appointments within insurer timeframes
• respond to inquiries from patients, referral sources, payors, and coworkers
• coordinate referrals and outreach
• monitor performance measures and improvement opportunities.
