What you'll do

• Provide physical therapy treatments in patients’ homes according to the licensed Physical Therapist’s plan of care
• monitor and report changes in patient status
• communicate with patients, caregivers, and interdisciplinary team members
• instruct patients and caregivers in exercises and assistive-device use as directed
• assist with care-plan updates, documentation, case conferences, and discharge planning
• travel to patients’ homes within the assigned territory.
