What you'll do

• Support individuals’ personal goals
• lead activities that foster independent living skills, healthy habits, and community participation
• assist with activities of daily living while promoting dignity and choice
• accompany individuals to volunteer sites and community activities
• complete documentation and progress notes
• collaborate with team members, families, and supervisors.
