Service Coordinator

The Service Coordinator role is onsite in Washington, PA, serving families throughout Washington County. Compensation is $43,000–$46,000 per year, depending on experience. The position supports families with children eligible for Early Intervention under Pennsylvania Act 212. The role exists to coordinate early intervention services and community resources around each child and family’s needs. Through assessments, family communication, service planning, and monitoring, the coordinator helps families access services and supports transitions to preschool services.

What you'll do

• Explain Early Intervention services to families
• coordinate screenings, assessments, and community resources
• coordinate with medical and health providers
• monitor child and family needs
• provide parent education and support
• facilitate and review Service Coordination Plans and Individualized Family Service Plans
• monitor service delivery
• explain parent rights and safeguards
• initiate preschool transition planning
• attend meetings and training
• and complete required documentation.

Why this role
