What you'll do

• Assist with bathing, dressing, grooming, and toileting
• support mobility, transfers, and basic exercises according to care plans
• help maintain a safe and comfortable home
• prepare meals and assist with feeding as needed
• take and record vital signs when trained and required
• assist with medication self-administration as directed
• report changes in condition
• document care
• participate in team meetings, training, and care coordination.
