Case Manager, Permanency

What you'll do

• Build collaborative relationships with clients, caregivers, placement providers, DCF, colleagues, and community partners
• provide in-home and community-based services, including caregiver-child observation, resource coordination, crisis intervention and safety planning, life skills training, transportation, and drug screenings
• advocate for clients
• attend court, school, and service coordination meetings
• complete case notes, assessments, court reports, and social histories.
