Per Diem Physician Assistant – Mobile Wound Care

The Per Diem Physician Assistant – Mobile Wound Care role is based in Fairfield, CA, with patient visits in homes and community-based settings across an assigned service area. Compensation is $100 per completed patient visit and $100 per hour for required training, meetings, and designated non-visit work. Dermacore’s stated goal is to bring safe, effective, patient-centered wound care to people who need it. The Physician Assistant assesses and treats wounds, monitors progress, and adjusts care plans when clinically indicated. The role also coordinates with patients, caregivers, referring agencies, physicians, and internal care teams to support continuity of care and escalate clinical concerns appropriately.

What you'll do

• Perform patient and wound assessments and develop individualized treatment plans
• provide wound care visits and procedures within scope, training, and privileges
• manage wound-related recommendations and orders
• monitor wound progression and adjust plans when indicated
• complete timely clinical documentation, with same-day charting expected
• communicate with patients, caregivers, agencies, physicians, and care-team members
• collaborate with clinical services, care coordination, and medical records
• escalate concerns or recommend a higher level of care when necessary
• participate in clinical education, quality initiatives, and team meetings
• comply with clinical standards, HIPAA, and applicable regulations.

Why this role
