Home Care Coordinator

As part of the Interdisciplinary Team, the coordinator assesses home-care needs, helps develop and update individualized care plans, and coordinates services so participants can receive safe, effective, person-centered support at home.

What you'll do

• Conduct in-home assessments and home safety evaluations
• collaborate with participants, caregivers, and family members
• contribute to care plans with the Interdisciplinary Team
• coordinate home-care services
• review notes and documentation for service gaps or concerns
• maintain service authorizations and compliant records
• support quality improvement and compliance activities.
