Primary Care Physician (PACE)

Provide primary medical care to PACE participants, including assessment, care plan development, and direct care. Participate as a member of the interdisciplinary team, attending assessments and morning updates. Refer participants to specialists and manage their care. Oversee and manage participants' use of medical specialists and inpatient care.
