The navigator connects people experiencing housing instability with housing options and community support, coordinating with healthcare providers, social service agencies, and landlords. The work supports housing stability and improved health outcomes through individualized guidance, advocacy, and follow-up.

What you'll do

• Assess housing needs and barriers
• collaborate on individualized service and housing plans
• provide referrals and linkages to resources
• conduct program assessments and 90-day reviews
• maintain knowledge of housing resources, subsidies, and community programs
• advocate with landlords, housing authorities, and service providers
• follow up on housing stability
• coordinate with healthcare providers
• document interactions and outcomes
• participate in community outreach and education.
