What you'll do

• Oversee daily community care program operations, including scheduling, compliance, and care coordination
• supervise RNs, CNAs, and home health aides
• ensure compliance with Medicare, JCAHO, and state standards
• manage patient intake, care plans, and interdisciplinary communication
• monitor program metrics and drive quality improvement
• coordinate with referral sources, discharge planners, and community partners
• assist with recruiting, onboarding, and performance management.
