Health and Wellness Director

What you'll do

• Assess residents' health, functional, and psychosocial status
• initiate individualized service plans and manage care
• establish and maintain the medication program
• oversee hiring and employment actions for licensed nursing, medication, and care staff
• train, supervise, and develop staff
• communicate with residents' families about health care needs
• maintain departmental policies, procedures, licenses, records, and charting
• represent the community to outside agencies.

Why this role

Requires an active LVN/LPN or RN license in good standing, at least four years of geriatric nursing experience, and at least two years in a supervisory or management role. RN preferred; memory care experience preferred. The posting states capacity to be on-call 24/7.
