Working under the Director of Clinical Services with a multidisciplinary team, the role develops aftercare plans and connects clients with providers, family members, and community resources.

What you'll do

• Develop client discharge and transfer plans
• coordinate referrals and ongoing care for housing and therapeutic needs
• document case management notes, assessments, referrals, and follow-up in clinical records
• obtain relevant prior records
• collaborate with clients on written, recovery-oriented aftercare plans
• provide case consultation with the multidisciplinary team and direct recovery support.
