What you'll do

• Provide medication reconciliation, dose modification or optimization, laboratory test ordering, and adherence counseling within approved protocols
• collaborate on individualized care plans
• monitor medication regimens, therapeutic goals, adverse effects, and interactions
• recommend therapy optimization and cost savings
• participate in quality improvement and clinical outcome work
• provide clinical justification for prior authorizations and appeals
• educate patients and promote medication safety.
