Personal Care Attendant

The Personal Care Attendant is an onsite, full-time role in Compton, CA. Compensation is not specified. Habitat Health provides coordinated medical and social care through its Program of All-Inclusive Care for the Elderly (PACE), supporting older adults in centers and homes. The attendant supports participants’ daily living, safety, social, and recreational needs in the PACE center and occasionally at home. The role includes personal care, activities of daily living, meal service, observation and documentation, safety checks, and support for participant activities and appointments.

What you'll do

• Assist with hygiene, feeding, ambulation, and other daily living activities
• prepare and plate meals according to dietary instructions
• record care information in the EMR
• maintain participant surroundings and care-unit supplies
• occasionally provide homemaking services at home
• accompany participants to appointments
• observe changes in condition
• monitor wandering alerts
• support activities
• conduct daily safety checks.
