What you'll do

• Complete individualized patient care plans and care coordination in the electronic medical record
• contact patients to provide support and manage program compliance
• document client communications
• communicate emergent or life-threatening needs to the primary care clinician or designee
• keep the team supervisor informed
• coordinate across organizational teams and partner facilities
• follow documentation and escalation protocols.
