What you'll do

• Plan and implement activities using the HEART philosophy
• assess residents daily to determine programs aligned with individual care plans
• facilitate socialization
• promote activities using the Staying Connected Form
• provide input into assistance plans
• coordinate decorations for events, seasons, and holidays
• plan weekly outings
• request supplies
• assist during meals
• monitor and direct residents on the neighborhood
• assist with transportation for outings and group trips
• help prepare activity calendars.
