What you'll do
- Learn Care Partners’ services and California Medi-Cal programs including CalAIM and Enhanced Care Management
- educate prospective clients and assess service eligibility
- coordinate with community organizations and respond to program inquiries
- complete screenings, authorizations, and scheduling
- coordinate handoffs with Lead Care Managers
- handle inbound and outbound calls
- maintain records and track call volume and outreach.
