What you'll do

• Assess patients and collaborate with the care team on plans of care
• make referrals based on identified needs
• teach self-care and promote health and independence
• supervise LPNs and home health aides
• complete timely, accurate documentation
• communicate patient progress
• plan and provide staff education
• perform assigned on-call responsibilities and services.
