The Clinical Care Specialist role is onsite at the clinic at 77 Broadway Street in Buffalo, NY. The stated schedule is full-time, Monday through Friday. Compensation is $19.50–$21.00 per hour. Horizon Health Services provides mental health and substance use services in the community. The specialist supports patients and multidisciplinary clinical teams through care coordination, records and data monitoring, patient outreach, and clinic response. These activities help patients navigate follow-up with behavioral health and medical providers and support the treatment team’s work.

What you'll do

• Coordinate care by relaying medication status and following up on releases, paperwork, reports, and applications
• conduct toxicology tests
• respond to clinic calls and walk-ins in crisis situations
• review and flag patient records
• support approved educational and process group activities after training and credentialing
• monitor clinical and non-clinical data
• provide patient support and triage
• assist with outreach and engagement
• attend Clarence Treatment Court weekly.
