What you'll do

• Oversee daily Social Services Department operations, procedures, staffing, records, and staff training and supervision. Complete or coordinate psychosocial assessments and individualized care plans
• participate in interdisciplinary care planning. Support resident rights and respond to grievances and family concerns. Coordinate admissions, transfers, discharges, referrals, and community resources. Collaborate with facility leadership and care teams, maintain regulatory compliance, and participate in quality assurance activities.
