• Conduct and coordinate MDS assessments and care planning
• submit assessments to the State Repository on time and accurately
• coordinate assessment review dates with the interdisciplinary care team
• update resident care plans
• monitor quality measures and Five-Star Ratings
• educate and collaborate with staff, residents, and families
• participate in surveys and inspections
• maintain confidentiality and patient privacy.
