Patient Navigator – Same Day Access

What you'll do

• Coordinate Same Day Access and scheduled intake
• connect clients with internal and external programs
• conduct initial in-person intake interviews and collect required information
• explain forms and financial policies
• communicate client information and next steps to providers and support staff
• maintain timely records and outcome data
• coordinate treatment plan updates and case management tasks
• collaborate with clients, families, providers, and community partners.

Why this role

Work hours are generally Monday–Friday, 8:00 a.m.–5:00 p.m., with one designated evening each week from 11:00 a.m.–8:00 p.m. A bachelor’s degree or equivalent education and experience in human services is required; case management training certification is due within six months of hire.
