Service and Support Administrator (SSA)

The Service and Support Administrator (SSA) role is onsite in Canton, OH. Compensation is not specified. The position supports the Stark County Board of Developmental Disabilities and its work coordinating services for eligible individuals. The SSA assesses each person's needs and preferences, develops an Individual Service Plan (ISP), and authorizes and monitors services. Through person-centered coordination with individuals, families, providers, and community partners, the role supports health, welfare, choice, and progress toward intended outcomes.

What you'll do

• Assess individuals' wants, needs, and risks using person-centered planning
• coordinate services with families, guardians, providers, and community agencies
• develop, monitor, and review ISPs and restrictive measures when necessary
• follow up with providers and monitor progress toward outcomes
• coordinate required assessments and community resources
• support provider choice and Medicaid eligibility
• document targeted case management and other services
• provide crisis assistance and report major unusual incidents
• train providers on ISPs and verify implementation.
