Bilingual Patient Engagement Representative

The Bilingual Patient Engagement Representative is an onsite, field-based part-time role in the Miami/Ft. Lauderdale area; compensation is $15–$25 per hour. Vitae Health Systems provides integrated clinical services to residents of skilled nursing facilities and supports facility care teams. The representative explains voluntary alignment to residents and authorized representatives, supports informed decisions and accurate form completion, and coordinates documentation and field updates with the value-based care team. The role helps carry out the program at the point of care and is nonclinical.

What you'll do

• Visit assigned skilled nursing and long-term care facilities
• explain voluntary alignment to residents and authorized representatives without pressure
• present and help complete forms under CMS guidelines and program protocols
• document visits and form outcomes
• coordinate with the VBC Coordinator and escalate cases outside standard protocol.
