Community Liaison

Job overview

The Community Liaison role is fully remote and based in Pocatello, ID, with a territory covering Pocatello, Idaho Falls, Twin Falls, and Ketchum. Pay is $50,000–$60,000 per year, with a monthly bonus opportunity. Home Helpers Home Care connects seniors, people with disabilities, and families with in-home care services. The liaison develops referral relationships and educates community partners about in-home care. By coordinating outreach and follow-up across the territory, the role helps connect people with care resources and supports clients and referral partners.

What you'll do

• Build relationships with social workers, discharge planners, case managers, physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare professionals
• educate partners about Home Helpers services
• identify opportunities to support seniors, veterans, people with disabilities, and families
• develop referral partnerships
• collaborate with internal teams
• represent the organization at community events and healthcare meetings
• manage territory outreach and follow-up.
