Community Navigator

Job overview

The Community Navigator role is primarily field-based in Woodland, CA, and throughout Yolo County, with some flexible work-from-home options available. Compensation is $23–$25 per hour. Meals on Wheels Yolo County works to nourish and engage vulnerable older adults and people with mobility challenges. The navigator assesses participant needs and connects seniors with community resources and services. The role supports food security, social connection, and access to services through person-centered assistance, outreach, coordination, and accurate documentation.

What you'll do

• Assist with screening referrals and conduct person-centered assessments
• work with participants and the Community Health Worker to identify strategies and connect seniors with resources
• build relationships with seniors, caregivers, volunteers, colleagues, and community partners
• participate in outreach and promote programs
• maintain participant records and required documentation
• communicate with supervisors and colleagues
• fulfill mandated reporting responsibilities.

Why this role

Part-time schedule is 20 hours per week, Monday through Friday, 9:00 a.m.–1:00 p.m., with some flexibility. The role is grant-funded and contingent on continued funding; required training may be paid for by MOW Yolo.
