What you'll do

• Assist the Team Lead with daily clinical operations
• guide and mentor RN Case Managers and clinical staff
• coordinate patient visits
• monitor care for continuity, quality, and compliance with hospice plans
• support nurses with clinical questions and symptom management
• make nursing visits as needed
• review documentation and monitor completion of assessments, recertifications, care plans, and orders
• communicate patient changes with physicians, providers, patients, and families
• coordinate with interdisciplinary team members
• participate in clinical meetings and escalate patient-care concerns.
