What you'll do

• Build relationships through outreach and visits
• serve as a point of contact for healthcare and benefits navigation
• identify needs and connect members with services
• provide wellness and benefits education
• coordinate appointments, providers, and third-party services
• support hospital visits, discharge planning, and follow-up
• identify home safety needs
• collaborate with Meta Care staff and diocesan contacts
• maintain confidential documentation.
