School Case Manager

The School Case Manager works primarily onsite at a school in the Devil’s Lake, North Dakota area, in a 12-month position serving students and families referred by the school district. The case manager coordinates services that address children’s and families’ treatment needs and supports resilience and safety. The role assesses needs, develops and documents service plans, connects families with resources, advocates for youth, and collaborates across child-service systems using trauma-informed and trauma-sensitive practices.

What you'll do

• Coordinate tasks to meet service recipients’ treatment needs
• complete assessments of child and family needs
• establish service parameters with families or legal custodians
• document case management and treatment planning
• implement services and goals
• provide direction to team members when applicable
• advocate for youth and family resources
• maintain community contacts
• provide crisis intervention as needed
• integrate trauma-informed care and trauma-sensitive strategies
• screen for trauma exposure and related symptoms
• support continuity of care across child-service systems
• maintain caseload documentation and participate in staff and supervision meetings.
