What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine eligibility and appropriate levels of home health care
• develop, implement, and document plans of care
• educate patients and families on self-care, nutrition, and dietary needs
• determine service scope and frequency
• assess caregiver capacity
• provide preventive and rehabilitative nursing procedures
• apply infection-control precautions
• direct Home Health Aides and LPN/LVNs
• document progress
• communicate changes to physicians and other personnel
• monitor assigned cases for payer requirements
• and coordinate with patients, families, and care professionals.
