What you'll do

• Conduct outreach, education, follow-up, and engagement
• support screenings, routine assessments, prevention, and risk-factor tracking
• help youth and families navigate health and community resources
• facilitate referrals and follow-ups
• deliver group health education and youth development activities
• adapt and deliver age-appropriate curricula
• maintain records and reports
• support needs assessments, evaluation, and measurement activities.
