What you'll do

• Provide skilled nursing care, assessments, and interventions in patient homes
• complete admissions and ongoing assessments and update care plans with patients and families
• respond to medical or emotional crises with appropriate in-home visits
• administer medications and treatments
• coordinate with physicians, DME, and the interdisciplinary team
• educate patients, families, and caregivers
• maintain documentation and compliance
• support IPU or office operations as needed.
