SYNERGY HomeCare Northeast Metro supports clients in remaining safe, comfortable, and as independent as possible in their homes. The Care Professional follows each client’s individualized Plan of Care to provide personal assistance, mobility support, companionship, and a safe home environment. The role contributes to clients’ daily well-being while documenting care and communicating changes or concerns to the appropriate people.

What you'll do

• Provide respectful care based on each client’s Plan of Care
• assist with activities of daily living and mobility
• offer companionship
• provide permitted hands-on care
• give medication reminders only, not medication administration
• observe and report changes, safety concerns, incidents, or accidents
• document care
• maintain a clean and safe environment
• and work professionally with clients, families, supervisors, and colleagues.
