What you'll do
- Serve as the contact point for children, families, and service providers.
- Convene the team and ensure continuity of care.
- Assist in quality assurance activities, including case record reviews.
- Assist in intake meetings with families to identify strengths and needs.
- Facilitate planning, coordination, and implementation of Child and Family Care Plans.
