The Case Manager Registered Nurse (RN), Part-Time or Full-Time role is onsite in Kansas City, MO, with compensation of $38–$52 per hour. Blessing Hands Home Care provides skilled nursing care to clients in their homes. The RN assesses clients, develops and updates physician-directed plans of care, delivers skilled nursing services, and coordinates with clients, families, physicians, and the interdisciplinary team to support safe, compassionate care and client independence.
What you'll do
- Perform nursing assessments at admission, during visits, and at discharge
- develop and implement individualized plans of care under physician direction
- provide wound care, IV therapy, medication administration, and disease-management education
- communicate with physicians, clients, families, and the interdisciplinary team
- document care
- monitor responses and update plans
- educate clients and families
- comply with HCBS and CMS regulations
- participate in case conferences, quality assurance, and continuing education
- report incidents and changes in condition.

Why this role
Missouri RN license and current CPR certification are required. One year of clinical nursing experience is required; home health, hospice, or acute care experience is preferred. The schedule may include some weekends or evenings if required.
