What you'll do

• Assess participant needs and gather information for program plans
• help participants identify needs and goals
• communicate progress with interdisciplinary team members
• coordinate, deliver, and document participant-centered services
• meet with participants at service locations
• support work assignments and training
• monitor progress and coach as needed
• communicate job expectations and business needs with site management.
