What you'll do

• Assess patients and families initially and on an ongoing basis
• develop, initiate, and update care plans
• provide nursing care, prescribed treatments, pain and symptom management, and patient and family education
• coordinate assigned caseloads and communicate with physicians, community health contacts, and interdisciplinary team members
• maintain clinical notes and attend interdisciplinary meetings.
