Registered Nurse (RN) Case Manager – Home Care

What you'll do

• Manage managed-care, workers’ compensation, and private-pay caseloads
• review and update care plans
• conduct in-home assessments and visits as needed
• monitor patient condition and coordinate follow-up
• track authorized services and requests for additional care
• communicate with patients, families, providers, payers, and adjusters
• maintain clinical documentation
• coordinate with scheduling, HR, and clinical leadership
• support field staff onboarding
• and assist with quality assurance of clinical notes and files.
