What you'll do

  • Conduct patient outreach, check-ins, and assessments by phone, text, video, or in person
  • assist with appointments, office visits, and social-service applications
  • coordinate referrals with health plans, providers, and multidisciplinary teams
  • use motivational interviewing, trauma-informed care, and harm reduction
  • document services in care-management portals
  • manage caseload priorities and follow-up
  • conduct community outreach and help organize or staff events.
