What you'll do
- Assist with intakes and referrals
- participate in multidisciplinary assessments focused on medical history, nursing assessment, health, nutrition, and safety
- help develop Individual Family Service Plans with families
- provide coaching-model services through home visits and child or parent groups
- monitor developmental milestones
- educate caregivers on health and safety
- complete progress notes, reports, assessments, and client records
- participate in community developmental screenings.
