The supervisor provides clinical guidance and escalation support to Community Health Workers while maintaining clear boundaries between non-clinical support and licensed care. The role strengthens care coordination, training, escalation protocols, and quality improvement with participating FQHC teams.

What you'll do

• Provide structured case consultation to Community Health Workers
• guide appropriate clinical escalation without independent diagnosis or treatment
• support chronic disease education and standardized workflows
• coach staff in motivational interviewing and goal setting
• develop escalation protocols with FQHCs
• support safety-event review, training, case conferences, and quality improvement
• collaborate with data and evaluation teams
• maintain HIPAA and documentation compliance.
