What you'll do

• Conduct medical assessments and physical examinations
• diagnose and manage acute and chronic conditions
• develop and update treatment plans
• order and interpret diagnostic tests
• manage medications and coordinate specialist consultations
• monitor physical, functional, and cognitive health
• collaborate with the interdisciplinary team
• coordinate care across settings
• educate patients and families
• maintain clinical documentation and participate in quality initiatives.
