What you'll do

• Build and manage professional referral relationships
• develop territory plans and conduct in-person and virtual meetings
• explain services, eligibility, intake, and referral pathways
• generate and document qualified referrals
• monitor referred patients through intake and authorization
• coordinate with Intake and Authorizations teams
• analyze referral activity and conversion
• participate in monthly targeted community or brand-awareness events and quarterly larger events.
