The Hospice Visit Physician, Field role is an onsite field position based in Farmington, CT, covering Northern Connecticut. Pay is per visit; no numeric rate is specified. AccentCare provides hospice and palliative care for people with serious, advanced, or terminal illnesses. The physician evaluates patients for hospice eligibility and recertification, manages pain and symptoms, and supports patients and families with care options and advance care planning. The role also coordinates with interdisciplinary teams and attending physicians to support continuity of care, and includes an on-call rotation with 24/7 availability when assigned.

What you'll do

• Evaluate patients for pain, symptom management, hospice eligibility, and recertification
• counsel patients and families on care options and advance care planning
• order tests, initiate treatment, and conduct face-to-face visits as needed
• serve as attending physician when appropriate
• coordinate with interdisciplinary teams and attending physicians
• manage medications
• complete timely, compliant clinical documentation
• participate in on-call rotation and provide 24/7 availability when assigned.
