What you'll do

• Provides physical therapy services according to a physician’s plan of care.
• Evaluates patient function and assists in developing individualized care plans.
• Educates patients, families, and caregivers.
• Translates prescribed exercises into functional activities.
• Recommends modifications to equipment and home environments.
• Collaborates with physicians and interdisciplinary team members.
• Documents clinical notes and progress reports.
• Supports and motivates patients and families.
• Supervises Physical Therapist Assistants.
