What you'll do

• Conduct initial psychosocial, emotional, spiritual, and bereavement assessments
• help develop and implement care plans
• participate in interdisciplinary team conferences
• document visits, calls, and referrals
• reassess patient and family needs
• provide counseling and crisis intervention when indicated
• advocate for patients and families
• coordinate referrals with community agencies
• participate in the Bereavement Team as assigned.
