The role supports patient outcomes through assessment, treatment, education, monitoring, and discharge coordination, including care in a unit that focuses on stroke and cardiac care.

What you'll do

• Coordinate multidisciplinary plans of care
• provide patient education and support
• administer medications and manage IV therapy
• perform procedures including dressing changes and blood glucose monitoring
• monitor responses to interventions and respond to acute changes
• collaborate with case managers on discharge planning and follow-up care.
