What you'll do

• Assess patients’ and families’ psychosocial, financial, environmental, and community needs
• assist with advance directives, funeral and discharge planning, family counseling, and changes in level of care as needed
• provide end-of-life and grief counseling
• and coordinate care through Interdisciplinary Group meetings and planning.
