The Social Work Care Manager - Palliative Care is an onsite role based in Billings, MT. , and this position supports the Palliative Care Team at St. Vincent Regional Hospital to provide high-quality, patient-centered care in the acute hospital setting. This role exists to perform psychosocial assessments, develop and implement care plans, and provide clinical counseling and therapeutic interventions. It contributes to the team by collaborating with patients, families, and healthcare providers to ensure effective care coordination and support for mental, emotional, and behavioral health needs.

What you'll do

• Evaluates patients for transition planning, mental health, substance use, and goals of care.
• Develops and monitors care plans, addressing social determinants of health and community resources.
• Uses motivational interviewing and therapeutic techniques to promote mental health care.
• Provides brief individual, group, and family therapy.
• Identifies mental, emotional, and behavioral disorders and connects patients to services.
• Facilitates clinical goals of care discussions.
